Terms of Service

These terms are an agreement between you and Affirmed Medical, PLLC ("Affirmed Medical," "we," "us"). They govern your use of this website and of the booking, payment, and account features on it. By using the site or booking an appointment, you agree to them. If you do not agree, please do not use the site.

Everything on this website, including our guides, frequently asked questions, and figures, is general information about hair loss and platelet-rich plasma (PRP) treatment. It is not medical advice or a diagnosis, and it does not replace an examination by a physician who knows your history. Results differ from person to person, and nothing on this site is a promise that a treatment will work for you.

Reading this website, writing to us, or booking a consult does not by itself make you our patient. A physician–patient relationship begins when a physician at Affirmed Medical evaluates you.

If you think you are having a medical emergency, call 911 or go to the nearest emergency room. Do not use this website, email, or text messages for anything urgent.

You must be at least 18 years old to book an appointment or join a membership through this site, and the information you give us when you book must be accurate and your own. You may book for someone else only if you have their permission and the legal authority to do so.

When you book a consult, we charge a consult fee of $70. If you go on to join a membership, the fee is applied toward it. If you do not, the fee covers the consult and is not refunded.

You may move or cancel a consult yourself, using the link in your confirmation, up to 24 hours before it starts. Cancelling that way refunds your fee in full to the card you paid with. Inside 24 hours, please call or write to us instead.

If you miss your consult without cancelling it ahead of time, you forfeit the fee.

Card payments on this website are handled by our payment processor. Your card number goes directly to the processor, and we never store your full card number.

Treatment at Affirmed Medical is offered as a membership. The price and the treatments it includes are as shown on our website when you join and in the agreement you sign when you start.

A membership has no end date. It continues until you cancel it.

You may cancel a membership at any time by emailing info@feelaffirmed.com or calling 212-655-7534. When you cancel, you are responsible only for any balance remaining on the treatments you have received, valued at $600 per treatment. If you have paid more than the value of the treatments you have received, we will refund the difference.

If you miss a treatment without cancelling or rescheduling it ahead of time, we may charge a missed appointment fee of $100.

Before any treatment, your physician will explain the procedure, its risks, and its alternatives, and ask for your written consent. Those consent forms govern your treatment. These terms govern the website and the booking and payment features on it.

If you have a patient account, keep your sign-in details to yourself. You are responsible for what is done with your account, and you should tell us straight away if you think someone else has used it.

How we handle information you give us through this website, and the text messages and emails we send, is described in our Privacy Policy. How we use and protect your health information is described in our Notice of Privacy Practices.

Please do not use this website to break the law, to interfere with its security or operation, to collect information from it by automated means, or to make bookings you do not intend to keep.

The text, figures, illustrations, logo, and other material on this site belong to Affirmed Medical or to the people who licensed them to us. You may view and print them for your own personal, non-commercial use. Please ask us before using them for anything else.

Links to other websites are provided for convenience. We do not control those websites and are not responsible for what they say.

This website is provided "as is." To the extent the law allows, we make no promises about the website itself, including that it will always be available, error-free, or up to date. This does not change any duty of care we owe you as your medical provider.

To the extent the law allows, Affirmed Medical is not liable for indirect, incidental, or consequential losses arising from your use of this website. Nothing in these terms limits our responsibility for the medical care we provide, or any liability that the law does not allow us to limit.

These terms are governed by the laws of the State of New York. Any dispute about them will be heard in the state or federal courts located in New York County, New York.

If we change these terms, we will post the new version on this page with a new effective date. A change does not apply to an appointment you booked or a membership you joined before it was posted, unless you agree to it.
